On April 15, 2019, Jewel Stroman opened a CFSA case on herself by reporting abuse to the MPD Police after leaving her children home alone all night with then nine year old daughter while she was up in the community helping other families.

William Henderson admitted to being the person that called CFSA claiming to see family going in and out the building all day. Mother Jakia Carroll was not able to walk until Sept. 15, 2018 due to scar tissue issues. Medical records will show Mother going to different hospitals and denied medical treatment. Jakia Carroll laid in Quality Inn Shelter room 508 for two weeks unable to move from 8/5/2018 - 9/15/2018. DHS, TCP, Nor any other agency assisted the family.

Ms. Thomas testified to DC Superior Court Judge Matini, DC Attorney General Kaitlin Rancier, GAL Benjamin, Attorney Lofty for father, and Phillip Skillman Court Appointed Attorney for Mother Jakia Carroll that, "On October 10, 2018, Stephanie Thomas said she was contacted by Lana Lopez to do A Student Portfolio for Kemontay Carroll. She then stated that Lana Lopez contact her a second time and requested her not to complete the due process for the education review because CFSA was removing the child. The DC Superior Court Judge Matini came onto record to say that this is normal in cases where children is in danger.

10/12/2018 DHS William Henderson & Jasmine Smith Homeless Families in DHS Shelter

William Henderson resident in Quality Inn Hotel Room 517. William Henderson is recorded by Mother Jakia Carroll telling of DHS plot to kill the Mother. Mr. Henderson went into details about how his friend Martin Winchester III made an attempt on Jakia Carroll life and failed. Mr. Henderson told Ms. Carroll on the video that it is him and Jasmine Smith that called CFSA about her son education. Mr. Henderson and Jasmine Smith is allowed to go through the shelter sharing sex offense registry with the families telling the families that Jakia Carroll is a predator that molested her daughter.

At the hearing Jasmine Smith told DC Superior Court Judge that DHS, CFSA, and TCP told her she had to open a TPO when she was evaluated for neglect for her son having a black eye with DC CFSA.
